To make a claim for a Centrelink payment while you are in Canada, you can download the claim forms and information from this page, complete the forms and return them to your nearest pension fund or social security office. If you are having trouble downloading the forms and information, you can telephone Centrelink on our Telephone Us page or visit your nearest pension fund or social security office.

What forms do I need to complete?

To make choosing a claim form that best suits your personal circumstances easy, there are a few steps to follow:

Step 1

Download and complete the claim forms:

Claim for Australian Pension while you are in Canada

Go to the Claim for Australian Pension form
Go to the Income and Assets form

Step 2

For certain payments, the following forms also need to be downloaded and completed so that your claim can be accurately assessed:

If you are claiming an Australian Disability Support Pension

Go to the Job Capacity - Customer Information form
Go to the Treating Doctor's Report form

If you are claiming an Australian Carer Payment

Go to the Assessment for Carer Payment form
Go to the Health Professional Assessment for Carer Payment form

Step 3

Print and complete your downloaded claim forms with your personal details. You may need to download and complete some of the below additional forms that have been identified when completing your claim form. Return your completed forms to your nearest pension fund or social security office.
